HELP! What is the name and brand of this top water lure?

I had this top water lure for years. I caught hundreds of bass on it all over the South East US. It broke a couple years ago and I’VE LOOKED EVERYWHERE FOR A NEW ONE!! Any help will be greatly appreciated.
