Id fill this out if you weren’t asking for identifying information.
Word of advice, remove the need to input last names prob first too. That is not necessary info for this survey and will prevent people like me who are willing to help but not willing to give random people my info.
I have led many surveys in the professional world and I have never asked for that info.
