Betta Fish: Is my betta okay?

I've had him for 11 days now, and he was doing pretty well but today he looks like this. He also doesn't seem to be eating too well but it's hard. He won't eat anything I give him and sometimes eats bloodworms if they're floating down. There's slight ammonia but nothing too bad
