We recently had a betta in this tank die, and I’m not sure what we’re doing wrong. Does she look bloated? She was just fed, but maybe we’re feeding too much?
I know it could also be ammonia so I’m doing frequent water changes just in case.

Are you saying you had two bettas in a 5gallon tank or am I not understanding your post?
She’s very pretty. She looks like she could be a tad bloated. What type of food are you feeding, and how much? You could try fasting her for a day or two to help the bloat subside, or feeding frozen or dried daphnia helps clear impactions as well.
Also just wanted to point out that it looks like your water heater is a tiny bit above water level. Please make sure to have it submerged completely as instructed, or they can seriously malfunction and potentially explode. You can move it down the glass a little bit, or it also looks like you have space to add a bit more water which should cover it 🙂
Looks a little overfed to me so just lower your feeding amount
Frequent water changes could be too much depending if this is 5 gallons and if you have bacterial established in your tank well. Is it cycled? Heater?
