Late stage fungal infection, could be from any number of initial causes like spawning, injury from predation, mishandled by angler. It dies and turns into bugs that feed other trout. Cycle of life.

That’s a great example of why we all need to keep trout wet. I’m sure that ole boy “swam off just fine” after he was last released, too.
