It’s difficult to tell from the photos but the center/top of the tank is cloudier than the bottom and has been that way for months.
Held off changing the water again until a few days ago and chose to trim the sword plant.
Prior to it being cloudy the water went unchanged for an extended period and couldn’t test it so I gave it ~40%. Next day, it was real cloudy, it improved over time but would never completely dissipate!

Looks like a bacterial bloom, biofilm growth probably went nuts with all the fresh water. Should calm down if you just let it settle for a bit 🙂
Just be patient, it’s common for new set ups and goes away on its own.
