The only way I can get my lure straight is by exiting the hook at that point, leaving the jig head completely outside the body of the lure. Shouldn't it go all the way inside?

Not quite right, you should push the swim bait up onto the keeper portion of that jig head. When you try again overlay the jig head outside the plastic so you know where it should exit, don’t be afraid to push it up further onto the lead.
