Any tankmates? If so, please list with how many of each: 1 snail, 1 harlequin raspora (I used to have more years ago but this the only one still alive)(I also have no interest in adding more)
What do you feed and how much: aqueon betta pellets about 5 pieces
Decorations and plants in the tank: two fake mushroom houses, driftwood, some quarts rocks, looking to get live plants

He has very clamped fins, not good at all.
Whats his setup? Water perimiters? Any tank mates?
Can you answer the mod questions so we can help? The tank looks very dirty so the parameters are likely off and poisoning him.
Please invest in a liquid test kit and perform a water change at minimum. Hoover up the waste on the bottom with a siphon.
This looks like new tank syndrome setting in – essentially the toxins in the water have built up and without an established nitrogen cycle the fish is being burnt and affected by ammonia from the waste. The deaths of the other fish supports this.
