She cares deeply but admits she can't take care of him anymore. Where do I start?
There are no live plants in the tank other than algae. The water is always dropping. Do I just… Start from scratch?

First off, if you have an already established tank then go with that, if not then take the fish out and do a deep clean with the tank, and don’t change the filter, you might have to do a fish in cycle not too sure about that but most likely, then start putting driftwood and plants for the tank, I would recommend real plants rather then fake ones, and get some good quality food for the Betta.
Is there even a heater or sponge filter in there? 😢
And that aquarium is not a good representation of “she cares deeply for that fish”.
