Tank size : 5 gallons
Heater and filter? yes
Tank temperature: 80.4
Ammonia, nitrites, nitrates, and pH: 0, 0, 5, 8.2
How long have you had the tank? How long have you had your fish? Over a year
How often are water changes? What is your process? Every 2-3 weeks. Siphon 1-2 gallons of water out then replace with tap water and a splash of distilled water. Condition with prime.
Any tank mates? Ramshorn snails – about ten
How much do you feed and how much? 6-10 betta bites once or twice a day depending on if he acts interested in eating
Decorations and plants in the tank: Amazon swords, dwarf sag?, vallisneria, red tiger lotus, dwarf water lettuce, ceramic? Alien looking thing, fake log at bottom of tank, cacao pod, various botanicals, floating log with moss in it
He is also flashing.

Are you conditioning your tap water before putting it in the tank? And what kind of tests are you using?

You need to be using water conditioner not just “a splash of distilled water” this fish hasn’t been doing well for a very long time. Just like someone else said you shoulda asked for help a long long time ago. And if he’s been sick on and off for a WHOLE year you should have asked for help when it first started.

You need to get your Betta out of that immediately and put him in a another container preferably something like a 2 gal plastic jug, and with very little water,like 2 inches of water only so he can easily go up for air. But then you also need to add methilane blue, almond leaf, and a pinch of sea salt to this water. Also you need to do a salt bath right away. You water is clearly bad so a water change needs to be made as well.
