Could anyone tell me if these are roots? (the white things) they aren’t growing from the base of the plant though so i’m a little confused.

yes those are roots because that stem is producing new sword plants. If you cut those plants off the stem you can plant two additional sword plants in your tank.

Yes, swords spread via runners. They usually run along the substrate, but yours seem to be climbing the stairway to heaven.
