Does it have a name?!

I had three of this fly in my box for a few years and I have no idea where I picked it up. It’s become my go to when fishing my local water and it never disappoints. I’m down to my last one and I would love to know to source, give props, and purchase if possible. I’ve researched online and at my local shops, and no luck. Here’s what I do know:
– articulated
– weighted conehead with red eyes (eyes fallen off)
– collar like woolly bugger
– tail like a bunny leech
– krystal flash wraps around shank
– two tone black and red hackle off the shank

I’ve tried all of the criteria below when researching and still can’t find a match. Appreciate any info!
